US Health and Wellness, Inc. ("USHWN," "we," "our"), a Wyoming corporation, provides healthcare technology, coordination, and management services, and coordinates telehealth consultations delivered by independent, licensed clinicians.
We use your information to coordinate your care with licensed clinicians and pharmacies, process payments, communicate with you about your consultation and orders, operate and improve our platforms, and meet legal obligations. Your data is never sold.
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
How we may use and disclose PHI. For treatment (sharing your intake and records with the reviewing clinician and dispensing pharmacy), for payment (processing your consultation and medication charges), and for health care operations (quality, compliance, and coordination). Other uses and disclosures — including most marketing uses — require your written authorization, which you may revoke at any time. We may also disclose PHI when required by law.
Your rights. You have the right to: inspect and receive a copy of your health records; request corrections; request restrictions on certain uses and disclosures; request confidential communications; receive an accounting of certain disclosures; receive a paper copy of this notice; and be notified if a breach of your unsecured PHI occurs.
Our duties. We are required by law to maintain the privacy and security of your PHI, provide this notice, and abide by its terms. We maintain administrative, technical, and physical safeguards consistent with HIPAA.

USHWN is not a medical practice and not a pharmacy. We do not practice medicine, provide medical advice, prescribe, dispense, or ship medications. All clinical decisions are made solely by independent, licensed clinicians, and all medications are dispensed by U.S.-licensed pharmacies. Nothing on this site is medical advice. Consultations are available to adults 18 and older in all 50 U.S. states and the District of Columbia. Care is provided by asynchronous (store-and-forward) review of your intake by a licensed clinician; a live audio-video visit is used where state law requires it. The clinician independently decides whether treatment is appropriate; a consultation does not guarantee a prescription. Non-controlled medications only. You agree to provide accurate, truthful health information — care decisions depend on it.
The consultation fee is collected when you submit your intake and covers independent clinical review regardless of outcome. Medication is charged only after a clinician approves treatment. All charges are disclosed before you confirm payment.
Platform, coordination, and professional services for business partners are governed by the applicable order form or partner agreement. Partners are responsible for their own legal and regulatory compliance. Compliance-support services, including LegitScript application preparation and management, are preparation and advisory services only — certification decisions rest solely with LegitScript, and no outcome is promised.
To the maximum extent permitted by law, USHWN is not liable for indirect, incidental, or consequential damages arising from use of our services. We do not warrant uninterrupted or error-free operation of the platform.

You may cancel for a full refund of the consultation fee any time before a clinician begins reviewing your intake. Once clinical review has begun, the consultation fee is non-refundable — it pays for the clinician's independent review, whatever the outcome. A consultation does not guarantee a prescription, and refunds are not provided based on a clinician's independent decision not to prescribe.
Medication is charged only after a clinician approves treatment. You may cancel a medication order for a full refund of the medication charge any time before the pharmacy ships it. Once a prescription medication has shipped, it cannot be returned or refunded, consistent with pharmacy law. If an order arrives damaged or is lost in transit, contact us within 7 days and we will coordinate a replacement with the pharmacy.
Medications are dispensed and shipped by U.S.-licensed partner pharmacies. Temperature-sensitive medications ship in insulated cold-chain packaging by expedited carrier. Tracking is provided when your order ships. Shipping charges, where applicable, are disclosed at checkout.
Fees for business-partner subscriptions, builds, and professional services are governed by the applicable order form or partner agreement. Where no agreement states otherwise, work not yet begun is eligible for a full refund; once work has begun, fees for services already rendered are non-refundable and any unearned remainder may be refunded pro-rata.
